Homura's 'New Weapon/Wish'
Her shield ran out of sand, which as you know, means that it can't stop time anymore until she resets the timeline. Furthermore, her shield outright disappeared when Madoka did that... thing with the breaking of reality, but her magical girl clothes did not. Therefore:
1) Maybe she chooses not to use the shield anymore, since without sand, it's nothing but a hunk of steel attached to her arm; this would also imply it's either impossible to reset the timeline and get more sand, or that doing so would really not fix anything so she's not bothering.
2) Maybe she chooses not to use the shield anymore out of respect for Madoka and upholds her memory with the bow/arrow combo, an ability granted to her when Madoka fixed her Soul Gem and gave her her ribbon.
3) Maybe she did in fact make a new wish in this timeline.

One thing to keep in mind is that Homura likely would NOT have a new wish to make (or probably would not have WANTED to make a new wish, knowing it's still a mess), so it's more likely that her wish is still the same and she exists 'beyond spacetime' like she did in all of the timelines but the first. In this case theories 1 and 2 are more likely.
It's also very obvious that Homura remembers the events of the past and Madoka, and therefore couldn't have been reset into making a new wish.
Runes
See Runes of episode 12

Observations
* Once again, this episode used Connect as an ending song. However, there was no animation sequence, as instead the credits rolled in black on a white background.
** This episode doesn't have a proper opening either.

*This episode ends with a squence which shows first the back of Madoka, Kyouko, Syaka, Homura and Maimi followed by the silhouettes of all Puella Magi which appeared in the anime. Even those which were only shown in their witch form´s. After that Madoka´s ribbon falls and Madoka´s soul gem is shown for a second.

*

*

*

*

Speculations
*

*

Initial broadcast and delays
This episode was scheduled to air on 24 March 2011 but has been pulled due to the 2011 Sendai earthquake and tsunami. On April 10, 2011 it was announced that Madoka was scheduled to resume its broadcasting on the 21 of April 2011 and it would air the last two episodes back-to-back.

*MBS (Kansai area): 11th & 12th episodes on Thursday, April 21 at 26:40 (effectively Friday, April 22 at 2:40 a.m.)
*TBS (Tokyo's Kanto area): 10th, 11th, and 12th episodes on Thursday, April 21 at 27:00 (Friday, April 22 at 3:00 a.m.) TBS had preempted its airing of the 10th episode due to its news coverage of the earthquake.
*CBC (central Japan): 10th, 11th, and 12th episodes on Sunday, April 24 at 26:45 (Monday, April 25 at 2:45 a.m.).

*
This page lists those who have encountered Kyuubey and guesses about wishes. Note that these are speculations and not the character's actual wishes.

Madoka's Wish
Deus Ex Machina or "I wish Mami were alive"
Madoka wishes Mami back to life. This one is so easily corrupted, especially if the "Witches are fallen magical girls" theory proves true. You wished her alive Madoka, you never said as a magical girl!

Fight to save people
Madoka wishes to help everyone in need, from suffering people, to the fallen such as Mami, to the witches lost in grief. She becomes a magical girl with the potential to solve any problem, but neither she, nor Kyubey, nor other magical girls have any idea how her powers work, so saving the world is still easier said than done.
Procrastinating Until the End of the World
Madoka resists making a wish until the very end of the series, when the apocalyptic scenario from her dream comes into play, and she uses her wish to solve everything at once or cause a global reset. "I wish everything would go back to the way it was before I met Kyubey!"

The Human Instrumentality Project
Madoka wishes to end all suffering. Everyone gets hugged and turns into Morning Rescue.

Power to Defeat the Most Powerful Witch
Witnessing Homura fight against an impossible foe, Madoka simply wishes for power -- enough to instantly defeat even the most powerful witch.

Homura's Wish
<span style="color:#e00;">✖</span> Do you really want her dream to come true?
Homura might have used her wish to help someone else (Madoka?) and is the person Mami was referring to when asking Sayaka whether she wanted to make someone's dream come true, or be thanked for making it come true.
<span style="color:#e00;">✖</span> If you do, you'll lose everything
Homura might have wished to become the perfect girl and undo her previously unhappy life. She was a classmate and friend of Madoka, which is why she knows her way around the school. However, everyone's memories of Homura vanished because she wanted her old flawed, bullied, unpopular self to be forgotten.
<span style="color:#e00;">✖</span> "Anywhere but here!"
Because Sayaka wished to heal Kamijou, she received healing powers in addition to the powers all other Magi have.
Homura has a unique power as well - She can either teleport or move faster than the human eye can follow. This suggests that her wish had something to do with movement. One likely conclusion to draw from this line of reasoning is that she used her wish to run away from something, possibly an abusive family.
<span style="color:#e00;">✖</span> "I wish my Oneesan was an unstoppable magical girl!"
Rather than choosing to be a magical girl herself, she was wished into being a magical girl -- perhaps from someone who loved her, such as a little sister. The little sister, or someone similar (another member of the family? A friend?) specifically wishes that Homura would be unstoppable, and ends up becoming a magical girl alongside her, but then dies in battle later. Homura, however, has become "immortal". If this was so, this would make her the one Mami is talking about when she warns against making wishes for other people. It also explains why she doesn't die/become injured when Charlotte chomps on her, why Kyuubey seems to flipflop on his involvement in her wish, why Homura seems so mature (and claims to have seen many magical girls die in her lifetime), and why she doesn't appear to have any family (all of her family members have died from old age). (Original poster: The "immortality" bit of this might not apply since episode 6; every Puella Magi is effectively immortal if her soul gem is not destroyed. Not to mention this theory doesn't explain Homura's superspeed.)
<span style="color:green;">✔</span> "I want to redo what I did wrong."
:<span style="color:green">This wish was revealed in Episode 10.</span>
Homura wishes to avoid a fatal outcome and go back in time to redo everything.

Mami's Wish
"I don't want to die alone!"
Before she met Madoka and friends, Mami was all alone-- her apartment was empty except for her presence, and had no contact with her family. When Kyuubey made his offer, Mami wished to not die alone. As in many "deal with the devil" stories, the contract is upheld, yet in a twisted manner: Mami made new friends and immediately died before their eyes.

"Please help…"
:<span style="color:olive">This wish is shown in the manga version, which is not considered canon by many.</span>
In manga canon, her wish is shown to be simply "please help" after being questioned by Kyubey about her wish in a car accident.

Sayaka's Wish
<span style="color:green;">✔</span> Kamijou
:<span style="color:green">This wish was revealed in Episode 4.</span>
Sayaka has wished for Kamijou's arm to be healed. As a side-effect, the wish granted her the ability of fast recovering. Whether there are any downsides to this wish (such as the healing being temporary, Kamijou being killed shortly after his recovery, etc.) remains to be seen.

Kyouko's Wish
<span style="color:#e00;">✖</span> I want to eat
According to abandoned child theory, she once almost ended up starving to death and wished for food when Kyuubey appeared with his proposal. Since that time she does not have problems with hunger as long as she gets Grief seeds.

<span style="color:green;">✔</span> Someone believe my father
:<span style="color:green">This wish was revealed in Episode 7.</span>
Kyouko wished people to believe in her father's unorthodox teachings.

Wasn't this debunked in Ep 10? This speculah's main point is that the names have been somehow switched after something like a global reset, but none of alternate timelines feature swapped names. Homura and Madoka make a big deal out of calling each other by their given names, so the part about Homura being angry because Akemi is reserved for close people is completely debunked. The last suggestion doesn't fit with the rest of the theory and seems to only serve as how the example might NOT support the speculah. == Summary ==

This episode centers around Homura. Most of the mystery behind her origin and behavior in previous episodes is explained.

Timeline 1
In the original timeline for Homura, she was a normal human girl suffering from a heart disease. She is shown just released from the hospital and transferring to the Mitakihara Middle School, similar to the first episode. She differs in appearance to the Homura we know, though - she wears glasses and wears her long hair tied in two braids. Her personality also feels different - she is a lot more shy and clumsy.

Madoka, Sayaka and other new classmates try to befriend Homura, which makes her feel uneasy. During a talk with Madoka, she admits finding herself worthless, not unlike Madoka does in the timeline of other episodes. Homura's problems during math and sports classes (probably related to being out of school for a long time) contribute to her depression, which makes her get suicidal thoughts on the way from school.

Presumably because of these thoughts, she wanders into the barrier of witch Isadel. She is quickly saved by Puella Magi Madoka and Mami, with Madoka asking her to keep their jobs a secret from schoolmates. Later, during a visit at Mami's apartment, Madoka reveals that she's been contracted only for a very short time. Homura feels fascinated about her magical friends, similar to how Madoka acts in initial episodes.

Some time later, Madoka and Mami fight Walpurgis Night together, with Homura following them as a human. Mami is defeated quickly, then Madoka apparently manages to defeat the witch, but at the expense of her own life. Homura cries over Madoka's dead body, and accepts Kyuubey's proposal to become a Puella Magi, her wish being "I want to redo my meeting with Kaname-san. Instead of being protected by her, I want to protect her!" The contract is formed, with Kyuubey commenting that the wish has "improved entropy".

Timeline 2
Homura wakes up in a hospital, on the date she was released in the previous iteration, having a Soul Gem in her hand. After transferring to school, she instantly tells Madoka about becoming a Puella Magi, hoping for a good cooperation.

Madoka and Mami train Homura the Puella Magi, who turn out to be extremely clumsy, despite having very powerful ability of stopping time. Meanwhile, Homura reads online guides about making explosives - all this contributing to improving her skills. While still clumsy, she plays the most important role during the trio's - now all being good friends - fight with a witch Patricia.

The next scene shown is after the Walpurgis Night - after defeating the witch, Madoka's Soul Gem turns into a Grief Seed and forms a witch.

Timeline 3
The time goes back again, and Homura wakes up at the exact same place and time as previously. Scared and angried by the discovery of the true nature of the contract with Kyuubey, she decides to tell the other girls immediately. Sayaka, who in this timeline became a Puella Magi, too, doesn't believe her and accuses her of collaborating with Kyouko, whom apparently only Homura has met yet.

Sayaka also tells that she feels afraid of Homura's explosives, which makes Homura go to a meeting of yakuza gang and steal some firearms, all on frozen time. Since Sayaka turns into a witch, presumably for similar reasons as in Episode 8, Homura uses the firearms during the fight with her to protect Madoka, and also defeats her, even though Madoka and Kyouko try to make Sayaka human again in a fashion similar to Episode 9.

Upon seeing the true nature of Puella Magi herself, Mami breaks down and attempts to kill all the magical girls, including herself, in order to prevent their eventual transformation into witches. She kills Kyouko first and attempts to kill Homura, but before she can do so, Madoka kills her. The remaining two fight on the Walpurgis Night together, and defeat the witch, having their Soul Gems fatally tainted, though. Madoka heals Homura with a Grief Seed she was hiding, and asks Homura to prevent her from becoming a Puella Magi next time she'll go back in time. Homura agrees, and grants Madoka one more wish of hers - killing her to prevent her from becoming a witch.

Timeline 4
Upon waking up in the familiar hospital, Homura changes her presence by magically healing her eyes, wearing the hair down and taming her emotions - just how she looked in all the previous episodes. She warns Madoka at night (with Madoka recognizing her to some extent as she says "Homu...") and goes hunting all the witches.

On Walpurgis Night, the Episode 1 dream sequence is shown again. Homura watches Kyuubey persuading Madoka and screams to prevent her from forming the contract, but Madoka doesn't seem to hear it.

Off-camera, Madoka forms the contract and defeats the witch with just one shot, but becomes a witch herself shortly after. Kyuubey comments that she may destroy the whole Earth in near future. He has collected an enormous amount of energy from her already, though, so he doesn't care about it anymore.

Homura refuses to fight Madoka the witch, which make Kyuubey realize that she's from a different timeline.

Timeline 5
This is presumably the timeline of all the previous episodes. Homura insists of repeating that particular period of time (between her transferring and the Walpurgis Night), no matter how many times, until she saves Madoka. She kills Kyuubey, but he respawns in a new body, which she chases, but he is saved by Madoka, similar to Episode 1.

Speculations and Observations
* This time, the only change spotted in the opening animation was the addition of Homura and Kyouko to the group shot. The opening animation has been used as the ending sequence. (this episode has no proper opening)
* The calendar in Homura's room shows that she's getting discharged from the hospital on the 16th and returns to school on the 25th. From the format of the calendar, the 25th is apparently a Friday. The only month in 2011 that has 31 days and the 25th falling on a Friday is March. This means that the anime is set during the same time the final episode will be airing.
* It seems like in every timeline the last ones left to fight Walpurgis Night are Homura and Madoka.
**It is assumed that Mami, Kyouko, and Sayaka always die in every timeline.
***However, some evidence suggests that Kyouko's and Sayaka's absence on Walpurgis Night in the other timelines could be explained by Kyouko never arriving in town (or joining forces) or Sayaka never making her wish.
****Under this assumption we are lead to believe that Mami dies in every timeline.
* It is possible that an imprint or a residue of past timelines are still within Madoka's memory. One example is the dream that Madoka had in Episode 1 and that strange image static in Episode 8.
* According to Kyuubey, Madoka as a Witch has the power to destroy the world but it is of no concern to him as long as they meet their quota.
**It is speculated that because of the nature of Madoka's wish in Timeline 4, that wish gave her the power of not only destroying the Witch with one shot but that same power could destroy the world once she becomes a Witch herself. For that to happen Madoka will have to make the same wish on Walpurgis Night in Timeline 5.
*Homura's only way to get guns and weapons is by stealing them, it is assumed that she must have stolen them from a group of criminals.
*Given the nature of Walpurgis Night and the presence of gears and the "falling" figure of the witch, Walpurgis Night could be the witch form of Homura. Although Homura's magic is very limited, her experience in multiple time-lines makes the witch a formidable foe.
*In Timeline 3, the Grief Seed Madoka uses to heal Homura maybe Sayaka's Grief Seed. She might have obtained it after defeating Oktavia at the station. Madoka might have kept the Grief Seed until the end to remember Sayaka.
** The Grief Seed from Episode 10 looks different from that of Episode 8. However, this can be explained if the Grief Seed from Episode 8 is an immature form. Both Sayaka and Madoka's Grief Seed does not show dark parts as other Witches' Grief Seeds (except Charlotte's Seed which was partially dark since Charlotte had revived recently).
*It was speculated in Episode 9 that if Kyuubey's people have the technology to travel back in time, they could fix the entropy problem by reversing it. However, in Episode 10, we learn that Kyuubey can only grant the ability to travel back in time. He is only part of the process to create it, so they do not posses the ability itself. The power of time travel rests only within Homura because of her wish.
*Although the assumption is that the prologue from Episode 1 is from Timeline 4, when Homura was fighting the powerful witch during Walpurgis Night, that same Witch seems to differ in her shape in Episode 10 during the actual Timeline 4.
**Walpurgis Night from Episode 10's Timeline 4 has slimmer body and longer dress.
**Walpurgis Night of Timeline 1 is also slightly different from other two.
*Homura's Past Timeline Experiences:
**Mami is mentally unstable, if she learns the truth she will kill everyone (she is unreliable as an ally).
**Sayaka refuses to believer or listen to Homura's advice. She will also be prone to making the same mistakes and become a witch (her fate is inevitable no matter what).
**From past experience, Homura believes Kyouko may be the only ally left to help her in stopping Walpurgis Night. However, she did not anticipate her actions that lead to her death since this didnt happen in previous timelines.
**Leaving Madoka alone or keeping her distance from her will not stop her from making her wish. Nor can Homura stop Walpurgis Night on her own.
**The only constant in previous timelines is that Madoka is the only one with a pure soul, she was the only one who called her a friend and trusted her with her life.
*Every time Homura tries to change the past she seems to make it worse:
**Original timeline: Mami and Madoka die like heroes and they are the only known casualties, Earth is saved (and there are no more threats to the planet). The Truth is never revealed.
**Second timeline: Mami dies as a hero, Madoka becomes a normal Witch.
**Third timeline: Sayaka becomes a witch, Mami dies but not before she becomes a psycho and kill Kyouko. Madoka still dies and becomes a witch but not before Homura kills her before that happens.
**Fourth timeline: Madoka becomes a powerful Witch that will destroy the Earth, everyone dies.
**Fifth timeline: Mami dies before learning the truth, Sayaka still becomes a witch and dies with Kyouko...

*Everytime Homura tries to change the past she seems to make it worst:
Summary

Summary

However, in Episdoe 10 we learn that Kyuubey can only grant the ability to travel back in time, he is only part of the process to create it, they do not posses the ability itself. The power of time travel rests only within Homura because she wished so.
*Everytime Homura tries to change the past she seems to make it worst:
Kyuubey is a "familiar," endued with the power to grant any wish for exchange of a contract-- for the person the wish is bestowed upon to become a Mahou Shoujo (magical girl). These magical girls are then tasked with the defeating of Witches. Kyuubey features prominently in the official release artwork first two episodes and is central to the current story.

Trivia
*People still argue as to how to write is name. Is it Kyubey or Kyuubey?
**His/her name is written in both Hiragana and Katakana as キュ(kyu)ゥ(u)べ(be)え(e). So the more acceptable Romaji for his/her name is Kyuubee/Kyuubey.
**On the other hand, the fifth commercial has his name romanized as "Kyubey", hence the argument.
**Apparently, it comes from the katakana of the english word Cute so it could also be Cubey.
***Moreover, the shape of the shadow in Episode 5 might suggest that Kyuubey originate in the mythological creature "Kyuubi no Kitsune"(Nine-tailed fox).
**Episode 8 leads to think that his name comes from 'Incubator', which romanization would be 'inkyubeetaa'

Speculations
*When Kyubey get shot by Homura in Episode 8, another one appears and eat the dead body. It could either be another instance of 'Kyubey' (as expected in some speculations), or the same Kyubey that respawned. After all, cats are supposed to have 9 lives, so what about him?
**Kyubey said that he has plenty of spares.
*Many fans believe Kyuubey is evil and believe he/she should not be trusted.
*According to the same theory, it is likely that Kyuubey is the messenger of a greater entity that has its own agenda.
*Some think Kyubey's real form is that of a red devil and his/her white fur acts as a cover as indicated by tears rather than bleeding in Episode 1.
** Additionally, when he gets shot full of holes in Episode 8 there are small drops that appear to be blood, but they do not spread out after he is hit. He also appears to have no bones or organs.
** Gen Urobuchi's tweet "Yeah, that's terrible! We still haven't shown even a single scene with bloodshed!" (まだ今んとこ流血シーンなんてないのに、ひどいよね！) may refer to the fact that Kyuubey doesn't bleed.
Summary
Pain Reduction
The episode begins with Sayaka in her room, outraged at the fact that Kyubey lied to her and Madoka about the relocation of a Puella Magi's soul upon making the contract. Kyubey explains that he offered her the contract, and she accepted: in return for a wish, Sayaka has dedicated her life to fighting witches. Because she never asked about the details, Kyubey saw no need to tell her. Kyubey adds that because a Puella Magi's consciousness is separated from their bodies, they are also able to endure pain that would break a normal person. Using magic, he inflicts pain on Sayaka as a demonstration of how the pain would be like if her soul was not transfered into the soul gem. Ominously, he notes that though Sayaka can completely ignore any pain if she so chooses, doing so can drastically reduce reaction time to injuries.

Sayaka's Absence
The next day, Madoka notices that Sayaka is absent from school and begins to worry. During lunch break, Madoka meets with Homura on the roof of the school and questions her why she never told either Madoka nor Sayaka about the true nature of the Soul Gem. Homura explains that she had tried to do so before with others, but that no one believed her. She again advises Madoka to avoid involving herself with Sayaka, and tells her that she should not confuse gratitude with responsibility; even though Sayaka has saved Madoka's life, Madoka is not in debt. Homura explains that when a Puella Magi makes a contract with Kyubey, they are receiving a miracle that may well be worth more than a life. Even if Sayaka had spent her entire life caring for Kamijou, there would have been no chance of him returning to play the violin. Kyubey grants the impossible, and thus a Puella Magi's side of the contract is dedicating the rest of their life to fight. Madoka asks if there's any way for it to end, and Homura bluntly tells Madoka to give up on her friend. Madoka wonders aloud how Homura can be so cold, and Homura responds introspectively, saying, "Perhaps it is because I am no longer human."

Kyouko's Backstory
Meanwhile, as Sayaka is laying in her bed, tortured by her emotions over Kamijou, she receives a telepathic message from Kyouko who is outside her house, holding a bag of apples. Kyouko tells her that she wants to talk. Sayaka complies and the two head towards an abandoned church. When they arrive at the church, Sayaka asks why Kyouko brought her here. Kyouko offers Sayaka an apple but when Sayaka throws it away, Kyouko angrily walks over to her and lifts her up by the collar, telling her not to waste food. After Kyouko calms down, she starts telling Sayaka about her past and why she chose to become a magical girl as well as the wish she made. Back when Kyouko was younger, her father used to preach at a church. However, her father's preachings began to stray from the original church's teachings, and he was excommunicated. This left Kyouko and her family poor, and Kyouko hated that no one listened to her father's words, so she made her pact with Kyubey to become a magical girl. Her wish was for people to listen to her father. People came in droves to listen to his preaching, and Kyouko fought witches in return for her wish, believing idealistically that she and her father would change the world through this way. Somehow, her father found out about Kyouko's wish, and that the people who came to worship did not do so out of faith, but out of magic. Calling her a witch who tempted men's souls, Kyouko's father snapped under the realization, and one night drank himself into a rage, and forced the family into suicide. Only Kyouko was left alive.

After Kyouko finishes telling her tale, Sayaka asks why Kyouko bothered to tell her. Kyouko tells Sayaka that she is headed on the same path as she once was when she became a magical girl, and she wants her to avoid those mistakes, and live for herself instead of for others. Sayaka apologizes to Kyouko for misunderstanding her, but informs Kyouko that she will never regret making a wish for someone else. She believes strongly that she will use her powers to save others, and keep those around her safe. In a moment of self-righteousness, Sayaka asks Kyouko where she purchased her apples. Kyouko is unable to answer. Sayaka guesses that the apples were stolen, and turns away, telling Kyouko that she cannot accept stolen food, and cannot in good conscience be thankful for Kyouko's offer in the first place. She then leaves the church, secure in her morals, leaving Kyouko in frustration.

Hitomi's Secret
Sayaka attends school the next day, and is joined by Madoka and Hitomi. Madoka worries, but Sayaka assures her she's fine. Sayaka's close friend Kamijou is also in attendance for the first time after being discharged from the hospital. Madoka says that Sayaka should go and see him, but Sayaka declines. After school, Hitomi asks Sayaka to meet her at a cafe where Hitomi tells her that she has had a crush on Kamjiou for a long time. This startles Sayaka, but Hitomi says that she will give her one day to confess, because she knows Sayaka has feelings for him too. Though if she cannot confess in one day, Hitomi will do so.

Comforted?
As night approaches, a depressed and confused Sayaka is heading out to hunt witches. She finds Madoka waiting outside. Madoka asks if she can come along, to keep Sayaka company, and this triggers Sayaka's held-back emotions. Sayaka begins to cry, saying that for a moment, she had regretted saving Hitomi's life before, and that she was no good as a moral figure of justice. She reveals that she feels as though she cannot ever face Kamijou with a shell of a body, and is emotionally torn and grieving over the apparent loss of both her humanity and her chance to be with the one she loves. After being comforted by Madoka, Sayaka appears to regain composure, and they head out.

The Witch Fight
Atop a scaffolding, Kyouko is idly watching a witch's barrier while eating an ice cream bar. Homura appears silently behind her, and asks her if she is going to allow Sayaka to take her kill. Kyouko at first brushes this off, telling Homura that since it's an actual witch and will drop a Grief Seed, that it isn't a waste like before. However, she notices strange behavior in the barrier, then curses, muttering that Sayaka is botching the fight. The scene switches to Sayaka and Madoka within the barrier; the witch is seated in a prayerlike position on top of a long arm holding a monstrance. Sayaka is then seen charging forth at the witch, Elsa Maria, which defends itself with snakelike animals and gnarled branches and vines. It fends off Sayaka, injuring her, and Kyouko soon comes to her rescue. Kyouko is about to attack the witch when Sayaka gets up and tells her to stay out of it. Sayaka again rushes forward, this time cutting past the vines, and reaches the witch. She slashes at the witch repeatedly, and begins to laugh insanely.

"He [Kyubey] was right. If I don't want it to, it doesn't hurt at all," are Sayaka's closing lines before she bisects the witch, referring to Kyubey's comment in the beginning of the episode about purposely numbing pain. The scene fades to black, and Madoka's voice is heard pleading, "Stop. Please stop."

Speculation
*Kyubey's talk in the beginning refers to the physical pain of injuries. However, Sayaka's last lines also imply the loss of emotional pain. Sayaka might use fighting against witches as a way of coping or closing away her emotional pain, and this might lead to risky decisions in the heat of battle.
*Sayaka feels as though she has already lost to Hitomi because she is, in her words, a zombie because of her soul's separation from her body. Ironically, the reason for that separation is her wish to heal Kamijou in the first place, and her side hope of being able to be with him. By wishing for Kamijou's recovery, all she has done is push Hitomi towards confessing her feelings to Kamijou, and emotionally block herself from being able to confess to him herself.

Observations
*The opening sequence reverted back to how it was in Episode 5, not featuring the colorful background anymore.
*Just before we get a close up of Sayaka's face in the church, there is an angel in one of the gothic windows that seems to be thrusting a sword (Sayaka's symbol) into Kyouko's silhouette.
*Kyouko's Soul Gem when she transforms has resemblance to the regilious symbol her father and the other believers had on themselves.

Kyuubey's proposition includes granting a wish in exchange for the person to become a mahou shoujo.

Homura's Wish
Do you really want her dream to come true?
Homura might have used her wish to help someone else (Madoka?) and is the person Mami was referring to when asking Sayaka whether she wanted to make someone's dream come true, or be thanked for making it come true.
If you do, you'll lose everything
Homura might have wished to become the perfect girl and undo her previously unhappy life. She was a classmate and friend of Madoka, which is why she knows her way around the school. However, everyone's memories of Homura vanished because she wanted her old flawed, bullied, unpopular self to be forgotten.
"Anywhere but here!"
"I wish my Oneesan was an unstoppable magical girl!"

In manga canon, her wish is shown to be simply "please help" after being questioned by Kyubey about her wish in a car accident.

Sayaka's Wish
Kamijou
As of episode four, Sayaka's wish has been confirmed; she has wished for Kamijou's arm to be healed. As a side-effect, the wish granted her the ability of fast recovering. Whether there are any downsides to this wish (such as the healing being temporary, Kamijou being killed shortly after his recovery, etc.) remains to be seen.

Kyouko's Wish
I want to eat
According to abandoned child theory, she once almost ended up starving to death and wished for food when Kyuubey appeared with his proposal. Since that time she does not have problems with hunger as long as she gets Grief seeds.

Someone believe my father
Revealed in episode 7 that, Kyouko wished people to believe in her father's unorthodox teachings.

Summary
*The episode starts off with Sayaka in her room, outraged at the fact that Kyubei lied to her and Madoka about the use of her powers of a Puella Magi. Kyubei explains that because Sayaka chose to become a Puella Magi, she should accept all the responsibilities that come with it. This leaves Sayaka feeling regret that she became a Puella Magi at all. The next day, Madoka notices that Sayaka is absent from school and begins to worry. During lunch break, Madoka meets with Akemi on the roof of the school and questions her why she never told either Madoka nor Sayaka why she knew all along about the true nature of the Soul Gem and its connection with the Puella Magi. Akemi merely replies by scolding Madoka for interfering once again, after numerous warnings that were given to her. She then goes on to say that Kyubei does not think of himself as a cruel being, he is merely the one who deals the contracts to the Puella Magi and gives them a choice whether to accept or not.

Meanwhile, as Sayaka is laying in her bed, thinking about the choice she made to become a magical girl, she receives a telepathic message from Kyouko who is right outside her house, holding a bag of apples. Back when Kyouko was younger, her father used to preach at a church, telling everyone the truths of the world, the words of righteousness and about the witches who were evil. However no one would believe him, everyone thought of him as a crazy cult leader. This left Kyouko and her family poor, they were without food and so Kyouko made a wish with Kyubei, she made her pact to become a magical girl. Her wish was for people to believe in her fathers words and to fight against the witches, the evils of the world. This was when Kyouko went public as a magical girl and fought the witches. She told her father that she had wished for people to follow him. This did not suit well at all with her father, she called Kyouko a witch and in a drunken rage, killed Kyouko's family, leaving only her alive.

After Kyouko finishes telling her tale, Sayaka questions why she told her. Kyouko tells Sayaka that she is headed on the same path as she once was when she became a magical girl. Sayaka apologizes to Kyouko for misunderstanding her, that she does have a sentimental side after all. Questioning where Kyouko got the bag of apples from, Sayaka says that since they were no doubt stolen, she can't accept one. She then exits the church, leaving Kyouko in frustration. The next day, when Sayaka attends school, she is joined by Madoka and Hitomi, wondering where she was the previous day. Madoka worries, but Sayka assures her she's fine. Sayaka's close friend Kamijou is also in attendance for the first time after being discharged from the hospital. Madoka says that Sayaka should go and see him, but Sayaka declines. After school, Hitomi asks Sayaka to meet her at a cafe where Hitomi tells her that she has had a crush on Kamjiou for quite some time now. This startles Sayaka, but Hitomi says that she will give her one day to confess, because she knows Sayaka has feelings for him too. Though if she cannot confess in one day, Hitomi will do so.

As night approaches, a depressed and confused Sayaka is heading out to get her mind off of everything by witch hunting. She is then confronted by Madoka who still thinks something is wrong. Sure enough, Sayaka is still fighting feelings of regret and questions whether or not she made the right choice to become a magical girl. After being held by Madoka, Sayaka comes to a resolve and regains composure, heading out to hunt the witch. Atop a scaffolding, Kyouko is approached by Akemi who asks her if she allowed Sayaka to hunt her "prey", the witch Kyouko was after. Kyouko curses Sayaka, calling her an idiot. Sayaka is then seen charging forth at the mysterious witch whose powers appear to be the manipulation of roots and vines. Madoka becomes worried after Sayaka is put in numerous predicaments. Kyouko soon comes to her rescue but is told by Sayaka not to interfere, that this is her fight. Sayaka, who has seemingly lost her sanity then rushes forth, attacking the witch, laughing, slashing mercilessly leaving Madoka to plead for her to stop.
