:They didn't fix it for the BDs, either... But in the manga, Sayaka is seen with the bat after the barrier dissolves (it's strange, though, since she has the bat in some panels, but not in others). So yes, it's most likely a production error. But it can be canonicalized that Mami's spell on the bat was permanent, and the magical entity (the bat) dissolved together with the barrier... hm, but the spell was made before entering the barrier... eh, so yes, it's most likely a production error. The "unknown shadow" are the woman's shoes. There's a shot of them before the girls enter the barrier, at one point there are red shoes being tossed around in the barrier, and after they defeat the witch there is another shot of them this time with tears and scuff marks.
Production error?
