Ultimate Madoka is the final form of Madoka Kaname. She is created when Madoka makes her wish with Kyubey. Her wish created a new world where no Magical Girls turned into Witches. She did this by taking every Magical Girl in history away with her just before their witchifacation.

Ultimate Madoka also appears in Magia Record as a playable unit.
