Homura took over the identity of that girl which had by concidence the same first name as Homura. In a school in Germany this theory was created:
Homura actually got killed before the begin of the story. Her soul gem was still intact and her soul was transfered into another body of a girl that either
1)picked it up
2)was just killed by the witch Homura took over the identity of that girl which had by concidence the same first name as Homura. That´s why Homura dislikes being called Akemi since it remembers her that the body belongs to another person (compare with Speculah: names)
Kyubey was not with Homura while the identity swap happend, so he deosn´t recognize her and states that she is an irregular.
Because of what happend to her she knows that the Puella Magi´s lives depend on the Soul Gems (maybe not the exact fact that the souls are in them). Due to this knowledge she could save Sayaka´s life in episode 6.
