Aniplex's Ani-Com magazine published a 100 question survey, filled out by Madoka, in December 2010.

It mostly concerns mundane things -- what do you wear when you go to bed, are you left-handed or right-handed, have you ever been confessed to, etc. These questionnaires were released while the anime was still in production, so certain answers are out of date and may differ from finalized canon.
