Tart Magica Witches
While a number of Witches appear in Puella Magi Tart Magica, the vast majority of them remain unnamed with the exception of the Witch form of Isabeau. Even Lapin's witch form remained unnamed until the release of Magia Record. Below are images of the various Witches that appear throughout the series in order of appearance.
Unnamed Witch 1
This is the Witch form of Lame, a magical girl under the control of Isabeau and her daughters. Its form is that of an owl, with the upper half of a woman instead of legs. It spirals out of the air when Elisa destroys its right wing.
Unnamed Witch 2
A bell-shaped Witch that Jeanne d'Arc, Catherine, and Riz Hawkwood encounter in the forest near Domrémy, France. A fairy-like Familiar lures Tart and Catherine to its Grief Seed, before they are both trapped within the Witch's wards. The Witch stays high up in its Labyrinth at first, before attempting to attack with a crushing fall.

Unnamed Witch 3
A Witch that briefly appears during Riz's time protecting Domrémy, France. It appears to be a sun-headed beast emerging from a floating crown.
Unnamed Witch 4
A Witch fought by Tart and Riz in the fortress of Vaucouleurs, France. Both it and its Familiars are made from countless vines. While the Familiars inhabit suits of armor, the Witch itself forms a giant humanoid shape after Tart shatters its wards.
Unnamed Witch 5
A crab-like Witch fought by Tart and Riz between Vaucouleurs and Chinon, France. On its back, it has a table set with a cloth, candlesticks, and utensils, but with a crying woman in place of a plate. The woman screams in agony which Tart shatters one of the Witch's claws.
Unnamed Witch 6
A Witch encountered by Tart and Riz between Vaucouleurs and Chinon, France. It only briefly appears in a flashback, and has the appearance of a frog stuck in the neck of a bottle. Behind it are many flying tadpoles, though whether they are Familiars or simply part of its Labyrinth is unknown.
Unnamed Witch 7
A flower-like Witch that Pernelle Flamel uses to test the abilities of Tart. She kills it quickly once Tart and Riz free her.

Unnamed Witch 8
This is the Witch form of Flèche, a magical girl under the control of Isabeau and her daughters. It has an appearance similar to hammerhead shark mixed with a crossbow. It is defeated by Tart with the help of Elisa during the Siege of Orléans.
Various Unnamed Witches
During the final battle against Minou and Lapin, Minou transported an army of unnamed magical girls to the battlefield. Once there, she causes them all to immediately transform into Witches which she used as an army against Tart, Riz, Melissa and Elisa.
Noticeably, these Witches have the original hosts' heads somewhere on their bodies.
