She is shown blowing through it, something you don't do with megaphones. the top of the weapon features three plungers, ostensibly to change the pitch and tone of the sound. Correction on Nagisa's weapon
Nagisa's weapon is not a megaphone, but rather is some kind of musical horn, most likely a trumpet.
